I am receipt of the (date) evaluation plan for my (child?s name). I give me permission for you to administer the (list tests), but not the rest of the ones on your list. I have investigated them and feel they are too unreliable.
In addition, I am formally requesting, pursuant to 20 U.S.C. § 1414 (a) and (b), that the plan reflect the following:
that the evaluator will meet with me to review (child?s name) entire history and will include the issues raised during that meeting in the report, and
that the evaluator will review samples of (child?s name) work and letters from professionals who have observed (child?s name).
I have one final concern. I have reviewed the credentials of (name of evaluator), the evaluator selected by the district to evaluate (child?s name). I am concerned that (name of evaluator) has no prior experience evaluating children with specific or suspected learning disabilities. Specifically, I do not believe (name of evaluator) is trained or knowledgeable about the specific tests to be administered, as required under IDEA 20 U.S.C. 1414 (b)(3)(iv). Therefore, I do not approve of the assigned evaluator, (name of evaluator), and request that an appropriate one be assigned, and that we be given proof of the evaluator?s qualification.
